Personal Health Information (PHI): Health-related details connected to your care, such as health history, care plans, therapy notes, or assessment results.
Health Information Custodian (HIC): Under PHIPA, Attunement Psychotherapy Centre and its regulated health professionals are HICs responsible for safeguarding PHI.

We use PI and PHI to:
• Provide psychotherapy and related health services.
• Coordinate and monitor your care.
• Schedule appointments and send reminders.
• Process payments and issue receipts.
• Maintain clinical and financial records in line with PHIPA and CRPO.
• Conduct internal quality assurance.
• Improve our website and online tools.
• Share educational content or invitations with your consent.

You may withdraw consent for any non-essential use at any time without affecting your therapy.

5. Confidentiality & Limits

Your information is confidential, except when disclosure is required by law, such as:
• Risk of harm to yourself or others.
• Suspected harm or neglect of a child or vulnerable person.
• Court orders or subpoenas.
• Reporting professional misconduct of a regulated health provider.

In such cases, we share only the minimum necessary information.

6. Telehealth & Electronic Communication

We offer secure video sessions through Jane and may use email or SMS for scheduling. While we use encryption and security measures, electronic communication carries some inherent risks, which you accept by choosing these services.

7. Cookies & Analytics

Our site uses first-party cookies and Google Analytics to understand usage and improve content. You may disable cookies, but some features may not work as intended.

8. Record Retention & Destruction

We keep clinical records for at least 10 years from your last appointment (or from your 18th birthday if you were a minor), as required by law. After this period, records are securely destroyed or anonymized.

9. Safeguards

• Encrypted Canadian servers (Jane).
• Locked storage for paper files.
• Role-based access control.
• Regular privacy and security audits.
• We do not intentionally store PHI outside Canada; if a service provider processes data abroad, equivalent protections will be ensured.

10. Disclosure to Service Providers

We work with trusted providers (e.g., Jane, payment processors, IT support, legal counsel) who are bound by confidentiality agreements. We never sell or rent your information.

11. Your Rights
